We also understand that professionals involved in this process have made assessments and given careful consideration to the suitability of a referral to Exceptional Care Ltd.
Our referral procedure therefore has been designed to minimise any anxieties surrounding the move. Wherever possible our aim is to collaborate with referring agencies in relation t a planned admission. Once a young person has been referred to Exceptional Care we will identify the most appropriate home for the young person based on the information provided by the local placing authority for the young person.
During our initial referral process where possible the Registered Manager of the home identified and the Responsible Individual will go out and visit the young person as part of our assessment.
Whenever possible our staff will go out and visit the young person before their admission to our home to help reduce anxieties and to meet some of the staff. if time allows the young person may wish to visit the home before being admitted (this can be as often as professionals feel necessary to aid the smooth transition into the home).
